Home and school partnership is imperative for any student to succeed regardless if the student has disabilities or not. However, when a child does have disabilities, especially if they are non-verbal, the importance of a home and school partnership increases greatly.

Parent/Teacher communication can happen in a variety of different ways. Some of the most common ways are:
  • Emails home
  • Weekly phone calls home
  • Communication journals
  • Daily forms sent home to parents





Some of the forms I have created and used are below.


This communication log allows the teacher to provide specific information to parents about what the child completed in school today and what kind of mood the child was in. It also allows room for other teacher comments and for parent feedback.


Parent notes are short little notes that I try to send home when I catch a student doing something good. It is also an easy way to send home a quick note about something as need --- for example -- "Remember picture day Friday!" These notes are double sided, on the back the parents sign and date and can write a question, comment, or concern before they send it back to school.


This is the parent communication log I use. It is great to be able to keep all parent contact information in one location and I also document every contact I have with parents and what we discussed. Each page is double sided.

This is a nice home/school communication forms as not only
does it allow the teacher to provide information to the parents,
but it also allows the parents to give a snippet of the child's
night. It wis also important for the parent to communicate
to the teacher if something happened in the morning that
may make the child be off that day. Another nice component
of these forms is that it provides pictures so that the child
could also have input in in filling out the forms with the parents
at home.

This is a simple form that shows the outline of the scheduled day. It allows the teacher to jot in a few notes about how the child did on each activity throughout the day.

It would be good to also have in a comment section where the teacher could write comments to the parents to explain the child's behavior if they did not receive a smilie face.

I would also recommend adding in an area for comments. The categories could also be changed to fit the need of the classroom or the student.
